เงื่อนไขการรับบริการ
1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำ E-Coupon มาแสดงเท่านั้น
2. E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้
3. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้
4. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น
5. อัตราดังกล่าว ไม่รวม ค่าพิมพ์แบบ การถ่ายภาพรังสีปกติและ/หรือ CT การเสริมกระดูกก่อนการฝังรากเทียมและข้อต่อพิเศษ
6. ชุดตรวจสุขภาพ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

เปิดบริการ ทุกวัน 09.00 - 20.00 น.

วันหมดอายุ E-Coupon
1. E-Coupon จะมีอายุการใช้ถึงวันที่ 31 กรกฎาคม พ.ศ. 2567
2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้
3. ชุดตรวจสุขภาพราคานี้ สามารถสั่งซื้อได้ตั้งแต่ วันที่ 23 มีนาคม พ.ศ. 2567 - 14 เม.ย.2567

หมายเหตุ
1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Shopee Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ
2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

1.Service is reserved exclusively for customers who present the E-Coupon.
2.This E-Coupon cannot be exchanged for cash.
3.E-Coupon is only applied to specific items as mentioned and changes cannot be made.
4.These packages are for outpatients who are physically dependent. 
5.The price mentioned does exclude the cost of regular radiographic imaging and/or CT scans as well as bone augmentation prior to implant placement and special joint procedures.
6.These packages exclude treatment fees or medications prescribed by the doctors if any. 
7.For further information or to schedule an appointment please contact Dental Clinic 2nd floor Dental Building Bangkok Hospital every day from 09:00 AM to 08:00 PM or Contact Center at 02-755-1335 or 1719.
8.Services are only available at Dental Clinic 2nd floor Dental Building Bangkok Hospital.
9.This price is for Thai citizens and expatriates only.

Expiration Date of E-Coupon:
1. The E-Coupon is valid until July 31 2024.
2. If the E-Coupon expires the hospital reserves the right to deny service access and the E-Coupon cannot be extended or refunded.
3.This package price can be ordered from March 23 2024 to April 14 2024.

Note:
1. In case customers request cancellation of the order after the Shopee Guarantee period without valid reasons from the hospital the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.
2. The receipt amount displayed is the actual payment received excluding any promotional discounts.
